Clinical Pharmacist

Job Description Summary The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with company policies, professional standards, and regulatory requirements. This entry-level position introduces practitioners to clinical pharmacy in an alternate setting and involves coordinating patient services, monitoring progress, and educating patients, caregivers, staff, and other professionals about medication use. Job Responsibilities Determine suitability of patients for home care, collect and organize all patient‑specific information, assess medical condition and prescribed medication therapy, and establish defined outcome goals. Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans, make appropriate medication therapy recommendations, and conduct nutritional assessments. Modify pharmacy care plans based on monitoring data, apply pharmacokinetic principles, and assess laboratory values. Provide medication‑use education to staff, other professionals, patients, and caregivers using effective counseling techniques. Ensure continuity of pharmaceutical care between home and other settings, prepare and dispense medications following policies, calibrate equipment, label appropriately, and comply with safety, quality, and regulatory standards. Manage use, maintenance, and troubleshooting of medication administration equipment, program and maintain equipment, advise others, participate in incident reporting, complete QAR documents, and monitor inventory discrepancies. Provide timely responses to drug information requests, formulate answers based on literature, attend CEU opportunities, and meet state CEU requirements. Manage investigational drug products per regulations, utilize support personnel, prioritize workload, organize workflow, and ensure accuracy of support personnel work. Comply with accreditation, legal, regulatory, and safety requirements in all activities. Maintain confidentiality of patient and proprietary information, observe ethical guidelines for safeguarding sensitive data. Supervisory Responsibilities No supervisory responsibilities. Basic Education and/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license issued by the State Board of Pharmacy. At least one year of experience in sterile IV admixture procedures. Knowledge of non‑sterile compounding, immunizations, and medication therapy management. Completion of an ASHP‑accredited Pharmacy Residency Program. Completion of an Advanced Pharmacy Practice Experience at an accredited school of pharmacy. Equivalent experience as approved by the Vice President of Pharmacy or Senior Vice President of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards, and evaluating customer satisfaction. Basic proficiency in Microsoft Word (document creation, editing, formatting, and collaboration). Basic proficiency in Microsoft Excel (workbooks, formatting, formulas, and data analysis). Basic proficiency in Microsoft PowerPoint (slide creation, editing, and presentation). Travel Requirements Willing to travel up to 5 % of the time for business purposes within the state. Preferred Qualifications & Interests (PQIs) Completion of an ASHP‑accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience at an accredited school of pharmacy. Experience in home care, hospital, or other clean‑room settings. Management Requirements At least one year of experience in indirect management of team members, including assistance in training, development, and assignment of work or projects to other team members. Compensation and Benefits Salary ranges from $97,113.10 to $161,862.67, determined by education, experience, and market data.
