Physical Therapist

Overview We are seeking a Physical Therapist to join our team! You will plan physical rehabilitative programs to improve patient quality of life. Responsibilities Develop and implement physical therapy programs for patients with strokes and neurological disorders that focus on rehabilitation for outpatient and home health treatment program. Work directly with patients in an outpatient, ALF and in home settings. Evaluate effects of therapy treatment and communicate patients' progress with their physicians and other healthcare providers. Record and document patient care services Collaborate with other team of therapists, nurses and physicians to achieve well-rounded coordinated care Qualifications Previous experience in physical therapy, rehabilitation in different settings, outpatients and home.
