The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting independence community involvement consumer rights personal cares daily living activities and regulatory compliance.

Key Responsibilities:
- Support members in community and recreational activities
- Assist members with physical social and personal needs
- Guide members in achieving personal goals and decision-making
- Create fun and engaging activities tailored to member interests
- Prepare and serve meals following dietary needs and preferences
Education/Qualifications:
- Must possess a high school diploma/equivalent and/or related work/life experience.
- Must be at least 18 years of age.
- Must possess a valid WI drivers license and ability to meet agency driving standards. Must provide proof of auto insurance.
- Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks and/or that of previous states of residence.
