Duties & Responsibilities BHH specialists provide care coordination, health promotion, patient and family support, and referrals to community services. Collaborate with the BHH Primary Nurse Care Manager and other team members to improve medical and behavioral health care coordination and outcomes. Work to reduce health care disparities; demonstrate competency in age-related or specialty issues and developmental needs for each population served. Assist in accessing medical and wellness services, including tobacco cessation, benefits, housing, treatment, legal services, and employment resources. Support money management and activities of daily living; assist patients with accessing necessary health care and transportation; may provide client transportation as needed. Provide high-level collaboration with medical providers for follow-up, labs, tests, and procedures to manage chronic conditions. Participate in hospital discharge processes to promote coordinated care transitions for behavioral health or medical admissions. Offer wellness interventions (e.g., nutritional counseling, smoking prevention/cessation, physical activity) and crisis intervention as clinically indicated. Serve as liaison between the agency, hospitals, community services, and family members. Maintain documentation in compliance with agency standards; participate in meetings, supervision, and training as assigned. Perform additional duties as required. Qualifications Education: BA in human services or related field required. Experience: Two years of experience in a mental health setting; experience with primary care providers preferred. Bilingual candidates strongly preferred. Licensure/Certification: Valid driver’s license. CPR/First Aid Certification within 6 months of hire. Compensation & Benefits Compensation: $19.28/hour minimum, commensurate with experience. Some locations and roles may have differentials or sign-on incentives. Part-time pro-ration may apply.
