Registered Nurse - Clinical Float - Medical/Surgical

Job Summary MO-SSM Health Saint Louis University Hospital 1201 Grand Worker Type: Regular. The Registered Nurse (RN), Medical Surgical Float Pool is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient’s hospital stay and coordinates care planning with other disciplines utilizing a patient/customer driven approach in a variety of Medical Surgical units. Must be highly energetic, flexible and motivated to support the success of Saint Louis University Hospital. Position Accountabilities & Performance Criteria Comprehensive Nursing Assessment/Reassessment Performs age‑appropriate admission or transfer assessment, obtaining input from family/guardian when appropriate. Accurately and completely documents findings. Performs assessment of post‑op / post‑invasive procedure patients. Assesses and documents education and discharge needs of patient and family on admission and throughout hospitalization. Provides patient reassessment documenting pertinent observations according to the patient plan of care, changes in condition, status and /or diagnosis, response to care, procedures, etc., and standards of care. Plan of Care Development Identifies short and long term goals based on patient care needs. Formulates nursing interventions to achieve desired patient outcome. Incorporates disease specific evidenced based practice into nursing care plan and other documentation. Nursing Interventions Collaborates with appropriate health team members for coordination of daily plan of care for assigned patients. Provides, coordinates and communicates patient care, including accurate handoff communication reports (bedside shift report, ticket to ride, SBAR, daily huddles, patient care conferences, etc.). Administers and documents medications accurately according to policies and procedures. Monitors, maintains and documents accurate IV fluids, blood, blood products and parenteral nutrition according to policies and procedures. Completes referrals as indicated by assessment data. Requests consultation for special needs, equipment, or information for patient and/or family. Provides patient/family education and discharge planning per documentation guidelines and protocol. Manages pain according to patient‑centered plan, including prevention strategies and involving patient/family in plan development. Assesses and reports potential abuse or neglect to physician, Risk Management and Social Work. Clarifies all physician orders as warranted, transcribes and implements them accurately and timely. Assists physician with procedures/treatments or delegates to Care Partner as appropriate. Recognizes changes in patient’s condition and takes appropriate nursing actions. Involves the family/guardian in decision‑making as appropriate and recognizes risks for patient. Completes and incorporates infection control bundles in daily care. Utilizes Fall and Skin Injury Prevention interventions and completes related audits. Documents and communicates nursing care and/or changes in patient condition; evaluates effectiveness of care and education. Identifies problems, gathers pertinent data, suggests solutions and works toward problem resolution. Reports variation from care/treatment following the occurrence reporting policy and procedures. Specialized Care for High‑Risk Patients Restraint Care – initiates/evaluates alternatives, applies restraints consistently, monitors response, provides individualized care during restraint period, consults peers for alternatives and 1:1 observation, documents restraint use and care thoroughly. Pain Management – assesses for pain on admission and during reassessments, incorporates cultural/spiritual beliefs, implements pain management techniques, includes patient/family in plan, considers massage, repositioning, immobilization and music therapy as alternatives. Abuse Assessment – recognizes abuse criteria, reports signs to physician, Risk Management and Social Work, takes appropriate action to support patient safety. Professional Accountability Adheres to all quality and performance standards, policies, procedures, protocols when implementing clinical and technical aspects of care. Participates in learning experiences that increase professional competence. Maintains currency in all hospital/unit information, communication, policies and procedures. Attends staff meetings and reviews minutes when absent, participates in committees, shared governance, work teams, and stays informed of hospital publications. Demonstrates ability to change and adapt to changing work demands, responds positively to change, supports peers and adjusts to unplanned workload. Provides supervision and teaching to students, new staff nurses; participates in mentoring and training. Ensures safety and infection control policies are followed, practices standard precautions, disposes of hazardous waste per guidelines, maintains a safe, clean, therapeutic environment. Completes mandatory annual requirements: current license, BLS, annual TB screening and influenza vaccination, ethics training, safety training, information security, abuse & neglect courses, bloodborne pathogens training, and competency reviews. Delivers service excellence to patients, families and colleagues by being courteous, responsive, confidential, collaborative, and resource‑wise. Incorporates performance improvement and evidence‑based practice in professional practice, identifies opportunities for improvement, contributes to data collection and evaluation of improvement results. Provides age‑appropriate care to infants, children, adolescents, adults, and geriatric patients, demonstrating knowledge of growth & development, age‑specific data, communication skills, and family involvement in decision‑making. Job Specifications Minimum Education : Licensed to work as an RN in the state of Missouri. Working Conditions : May be exposed to the risk of blood borne diseases; exposed to unpleasant elements; subject to varying and unpredictable situations; handles emergency; may work irregular hours. physical Requirements : Occasional prolonged standing/walking; lifting, pushing, transferring patients; reaching, stooping, bending, kneeling, crouching.
