Responsibilities

• Collect data relevant to residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team
• Contribute to a developmentally appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve the resident/family and healthcare team
• Recommend revisions to the plan of care after evaluating resident outcomes; arrange referrals as needed
• Collaborate with residents, families, and the Interdisciplinary Team to ensure comprehensive care and a safe environment
• Maintain current nursing knowledge and competency; participate in mentoring and orientation of staff; provide leadership under the RN
• Apply standard precautions and maintain safety when caring for adults and geriatric residents

Requirements

• Active NY LPN licensure
• BLS certification (American Heart Association or RQI)
• Minimum 1 year of LPN experience, preferably in a long-term care setting
• Ability to provide direct and indirect skilled nursing care to adult and geriatric residents
• Comfortable working under RN supervision and collaborating with an Interdisciplinary Team
• Capacity to work a 5x8 rotating schedule, including weekends and holidays
