Job Summary SIGN ON BONUS AVAILABLE for eligible Non-MGB employees: $10,000 FOR LICSW The Social Worker, imbedded in adult medicine, is responsible for performing social work assessments, interventions, crisis management, and relevant casework as needed for outpatients of the adult medical department. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving in collaboration with both medical and behavioral health providers on site. As a member of a multidisciplinary health team, they perform comprehensive and culturally considerate assessments and coordinate next steps in care that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Social Workers are responsible for compassionate care of patients that supports patient-centered plan of care for both individual patients and the medical community. Does this position require Patient Care? Yes Essential Functions -Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. -Formulates biopsychosocial assessment, disposition, and brief treatment plans. -Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. -Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan. -Responding to urgent and emergent needs of patients onsite in collaboration with medical and behavioral health team. -Providing relevant case management and care coordination relevant to behavioral and mental health needs. Qualifications Education Master's Degree Mental Health & Behavioral Medicine required or Master's Degree Social Work required Can this role accept experience in lieu of a degree? No Social Worker II Licensed Independent Clinical Social Worker [LICSW - State License] Clinical Experience in a medical and/or behavioral health setting 2-3 years required Behavioral Health Counselor Licensed Mental Health Counselor [LMHC - State License] preferred 3-5 Years related experience and experience negotiating, advocating, and navigating systems and demonstrated experience working successfully with people of various background and lifestyles in a manner that engages on-going self-reflection and continual learning required Knowledge, Skills and Abilities - Spanish fluency required - Ability to collaborate effectively with multiple teams - Demonstrates proficient clinical judgment and application of professional values and ethics. Integration of prior clinical experience into current practice. - Demonstrates proficiency to formulate assessment of risks, crisis intervention, psychosocial assessments, treatment plans, and the documentation of both in a timely manner. - Demonstrates sound clinical judgment and innovation in advocating for clients while maintaining professional boundaries. - Demonstrates proficient working knowledge of internal and external resources and refers appropriately. - Provides a range of evidence-based interventions. - Ability to advocate, coordinate, and continuously communicate with treatment plans with patients, family and the interdisciplinary team. - Incorporates social sensitivity. - If appropriate, utilizes supervision and consultation regularly and appropriately. - Actively seek assistance with complex cases and situations in a timely manner. - Engages in quality improvement projects, use data to measure progress. - Facilitate skill-based groups. - Knowledge, understanding and ability to negotiate and work in a complex organization. - Demonstrate the capacity to effectively communicate findings with a broader audience.
