If so, apply today!/ppWhy Join the Elara Caring mission?/pulliWork in a collaborative environment./liliBe rewarded with a unique opportunity to make a difference/liliCompetitive compensation package/liliTuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost/liliOpportunities for advancement/liliComprehensive insurance plans for medical, dental, and vision benefits/lili401(K) with employer match/liliPaid time off, paid holidays, family, and pet bereavement/li/ulpAs a Behavioral Health Licensed Practical Nurse youll contribute to our success in the following ways:/pulliVerifies the Plan of Care prior to each visit and provides care according to delegation by the RN, physicians orders, assessment data, and established standards and guidelines./liliIncorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes./liliCommunicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care./liliReports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate./liliImplements the plan for patient safety, using the patient, family, and community resources./liliAssists the patient in learning appropriate self-care techniques while delivering quality patient care./liliSuggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager./liliProvides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician./liliCollaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions./liliDocuments medication regimen and updates medication profile according to established procedures./liliTreats patients and caregivers in the highest and most effective manner./liliImplement all available actions to prevent avoidable hospitalizations and ER visits./liliProvides positive, supportive communication to physicians, patients, families, visitors, and agency personnel./li/ulpWhat is Required?/pulliHigh School Diploma or GED/liliGraduate of an accredited Certificate, Diploma, or Associate School of Nursing/liliCurrent State License as a Licensed Vocational / Practical Nurse/lili1 year of experience in a clinical care setting/liliExperience in a hospice or home health environment is preferred/lili50% travel required/liliAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment/liliValid drivers license and insurance and reliable transportation to perform job tasks/li/ulpYou will report to the Clinical Team Manager./ppThe base salary for this position is $33.00 to $35.00 per hour, based on the companys good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity./ppThis role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings.
