Contracted In-Home Occupational Therapist Albany, NY We’re looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. A successful candidate is highly empathetic, can juggle the needs of our clients, programs and business, and is eager to make a lasting impact. We’re a tech‑first team so comfort working with new systems and tools will be an advantage. A huge plus is if home modification work is part of your experience. Role Highlights Anticipated Start Date: 8/1/26 Employment Type: Contracted, minimum of 10 hours of weekly availability Location: In-Home Visits Key Responsibilities Conduct in-home assessments and functional evaluations Deliver personalized, evidence-based recommendations for home modifications Educate clients and caregivers on fall prevention and safe mobility techniques Engage clients and families in collaborative care planning Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives Maintain accurate and timely documentation through our electronic system Qualifications Degree in Occupational Therapy from an accredited program Active, unencumbered OT license 2+ years of clinical experience Experience in home health, aging in place, or disability services preferred Strong interpersonal and clinical reasoning skills Reliable transportation and valid driver’s license Proficiency with technology and digital documentation Ability to work independently while collaborating with a remote team More About Compensation Contractors are paid on a monthly basis, within 2 weeks of the previous month’s end.
