Astrana Health is actively seeking a motivated Nurse Practitioner / Physician Assistant in Houston to lead Annual Wellness Visits (AWVs). This hybrid role involves conducting visits in patients' homes as well as via telemedicine, essential for assessing health and improving outcomes. The ideal candidate will hold an active Texas NP or PA license, possess strong clinical skills, and have experience with direct patient care.
