If so, apply today!/ppWhy Join the Elara Caring mission?/pulliWork in a collaborative environment./liliBe rewarded with a unique opportunity to make a difference/liliCompetitive compensation package/liliTuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost/liliOpportunities for advancement/liliComprehensive insurance plans for medical, dental, and vision benefits/lili401(K) with employer match/liliPaid time off, paid holidays, family, and pet bereavement/liliPet insurance/li/ulpAs a Registered Nurse Case Manager, youll contribute to our success in the following ways:/pulliEnsures that all activities performed align with the vision of Elara Carings board of directors, executive team, and the leadership of the Hospice team./liliAssesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations./liliAssesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status./liliCommunicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care./liliCounsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care./liliTeaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.)./liliProvides appropriate support at time of death and perform bereavement assessment./li/ulpWhat is Required?/pulliGraduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing/liliCurrent State License as a Registered Nurse RN/lili1 year of experience in a clinical care setting/liliExperience in a hospice or home health environment is preferred/lili50% travel required/liliAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment/liliValid drivers license and insurance and reliable transportation to perform job tasks/lipYou will report to the Clinical Team Manager.
