Primary Care Physician Physician

Responsibilities Providing primary care to senior patients within a value-based care model focused on quality, prevention, and long-term outcomes. Managing patients with complex and chronic conditions through in-depth, longer visits (12-15 patients daily). Completing accurate, same-day documentation and managing clinical inbox communications. Coordinating care with specialists and team members, ensuring effective follow-up and care continuity. Applying Medicare risk adjustment principles via precise condition documentation. Qualifications & Skills MD or DO from an accredited program; board certified or eligible in Family Medicine, Internal Medicine, or Geriatrics. Active, unrestricted medical license or ability to obtain one in South Carolina. Preferred experience with senior populations, Medicare Advantage, risk adjustment, supervising Advanced Practice Providers, and proficiency with EHR systems.
