Job Requirements

POSITION SUMMARY

To be fully engaged in providing Quality/No Harm, Customer Experiences, and Stewardship by examines and provides remedial services for speech and language disorders. Devices vocal exercises and teaches proper breath control to improve speech. Performing interdisciplinary case management and providing appropriate age-specific therapy services.

PRIMARY ACCOUNTABILITES

1. Adhere to all Health First, Inc., Health First Home Care policies, and procedures, ensuring that all actions align with organizational and regulatory standards.
2. Conduct thorough assessments, including evaluations of home safety, pain, nutrition, fall risk, and potential abuse, upon admission, recertification, and as necessary for patient well-being.
3. Develop and implement individualized Plans of Care based on assessment findings and physician orders, adjusting as needed to support optimal patient outcomes.
4. Assist in the orientation of new associates and provide ongoing staff education to maintain high standards of patient care.
5. Ensure that all Home Care services meet State and Federal regulations as well as Joint Commission (JCAHO) standards to uphold the quality of care and compliance.
6. Actively engage in Quality Improvement initiatives to enhance patient care, safety, and service delivery standards.
7. Regularly evaluate patient outcomes, documenting changes in condition and progress, and communicating with the physician regarding any significant updates.
8. Supervise Home Health Aides at least every 14 days to ensure compliance with care standards and provide necessary guidance.
9. Accept all referrals regardless of geographic territory and respond to new referrals within two calendar days to ensure timely patient care.
10. Provide comprehensive case management, ensuring the Plan of Care is followed according to regulations, documenting any alterations within two days of the visit.
11. Inform patients and families of visit schedules in advance, notifying them of any changes to maintain clear communication and enhance patient satisfaction.
12. Utilize supplies cost-effectively, maintain productivity standards, and document all services accurately to support financial responsibility and operational efficiency.

Work Experience

MINIMUM QUALIFICATIONS

• Education: Master's degree in Speech-Language Pathology.
• Work Experience: One (1) year acute care experience with adult neurological population.
• Licensure:

1. Current Licensure in the State of Florida as a Speech-Language Pathologist.
2. Valid Driver's License and clean Motor Vehicle Record and maintained.

• Certification: American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Card prior to start date and maintained.
• Background Check: Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening.
• Skills/Knowledge/Abilities:

1. Good communication skills.
2. Ability to interact with other professionals, families, and patients.
3. Ability to decide concern appropriate therapy and patient care.
4. Ability to respond appropriately to life threatening situations.
5. Ability to assess, interpret data and educate patients and families of various ages and levels of understanding.
6. Ability to work autonomously.

PHYSICAL REQUIREMENTS

• Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, or crouching frequently.
• May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.
• May require moving in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders, or overhead.
• May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
• May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
• May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
