Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A direct hire opportunity is available for a Clinical Pharmacist specializing in home infusion therapy near Indianapolis, IN. This role is critical in managing complex intravenous (IV) and subcutaneous medication therapies for patients receiving treatment at home. The ideal candidate will have expertise in sterile compounding, clinical monitoring, patient education, and collaboration within a multidisciplinary healthcare team.

Schedule:

• Monday to Friday
• 8:30 a.m. to 5:00 p.m.

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions under sterile conditions, adhering strictly to state and federal regulations.
• Assess lab values and pharmacokinetic dosing (such as vancomycin), monitor Total Parenteral Nutrition (TPN), and adjust treatments as needed.
• Provide detailed counseling and education to patients and caregivers about specialty medications, infusion pumps, sterile techniques, and catheter care.
• Coordinate with prescribing physicians, home health nurses, and internal teams including intake, referral services, nursing, clinical services, and pharmacy operations to ensure optimal patient outcomes and reduce hospital readmissions.
• Maintain compliance with pharmacy regulations and standards (State Board of Pharmacy, HIPAA, DEA, Medicare, ACHC, URAC, OSHA).
• Manage pharmacy inventory, including refrigerated products, and ensure safe packaging and timely shipment of medications and supplies for home delivery.

Qualifications:

• Current pharmacist license in Indiana, in good standing.
• Minimum of 2 years’ experience in infusion specialty pharmacy.
• Experience with Intravenous Immunoglobulin (IVIg) is a plus.
• Familiarity with specialty therapy payer requirements, reimbursement procedures, and contract obligations.
• Knowledge of CareTend pharmacy system preferred.
