Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A clinical pharmacist role is available in the greater Louisville, KY area, focusing on home infusion therapies. This direct hire position offers an opportunity to apply specialized skills in a dynamic environment, supporting patients with complex intravenous and subcutaneous medication needs.
Location: Shepherdsville, KY (Greater Louisville Area)
Schedule:
- Monday to Friday
- 8:30 AM to 5:00 PM
Start: ASAP
Key Responsibilities:
- Prepare sterile specialty pharmaceuticals and IV solutions adhering to rigorous state and federal regulations.
- Monitor clinical parameters by assessing laboratory values, calculating pharmacokinetic dosing, and overseeing total parenteral nutrition (TPN) adjustments.
- Provide detailed patient and caregiver education on infusion pump usage, sterile technique, and catheter maintenance at home.
- Collaborate with physicians, home health nurses, and various internal teams including intake, referral services, reimbursement, nursing, and pharmacy operations to ensure integrated patient care and reduce hospital readmissions.
- Maintain compliance with applicable regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
- Manage pharmacy inventory (including refrigerated products) and coordinate packaging and shipping for timely patient delivery.
Qualifications & Desired Experience:
- Minimum of 2 years’ experience in infusion specialty pharmacy.
- IVIg (Intravenous Immunoglobulin) experience is a plus.
- Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Current and good standing Kentucky pharmacist license.
- Familiarity with CareTend system preferred.
