Primary Care Physician at St. Paul's Senior Services El Cajon, CA

Position Summary Provides primary medical management for St. Paul's PACE program participants in the El Cajon Clinic. Collaborates with other PACE physicians and providers to perform initial and semi‑annual assessments, additional physical assessments as needed, develop and implement care plans, evaluate physical complaints, provide treatment, and deliver teaching and education to participants and caregivers. Functions as a member of the Interdisciplinary Team (IDT) and rounds in contracted nursing homes, assisted living facilities, board and care facilities, and participants' homes. Job Details: Full‑time position. Typical hours are 8:00 AM – 4:30 PM, Monday‑Friday. Responsibilities Perform initial and semi‑annual assessments and additional physical assessments of participants as needed. Develop and implement appropriate care plans for St. Paul's PACE program participants. Evaluate participant physical complaints and provide appropriate treatment. Provide teaching and education to participants and caregivers. Function as a member of the Interdisciplinary Team (IDT). Round in contracted nursing homes, assisted living facilities, board and care facilities, and participants' homes. Inform the IDT of the medical condition of each participant, remain alert to input from other team members, participants' caregivers, and document changes in the participant’s medical record consistent with documentation policies. Basic Qualifications Certification as an MD with appropriate prescribing authority. Current driver’s license and proof of insurance. Minimum of 1 year of experience working with the frail or elderly. Medical clearance for communicable diseases and up‑to‑date immunizations before having direct participant contact. Must be able to clear fingerprints through DOJ and FBI in addition to a drug screen clearance. Benefits Health, dental, vision, and life insurance. Same day pay available.
