Overview Job: Occupational Therapist (OT) Home Health - Full Time or Part Time Location: Oakland Overview: We are seeking a compassionate, organized, and team-oriented Occupational Therapist (OT) to join our Home Health team in Oakland, Alameda. You will evaluate patients' health, create and implement care plans, and track progress. You will work closely with the Clinical Director and Clinical Staff in the field. Responsibilities Provide, coordinate and oversee skilled intermittent care to patients in the patient's home, including instruction and education of patients and/or caregivers to promote maximal functional status. Regularly evaluate and re-evaluate patient status. Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing as needed. Initiate, develop, implement and revise the POC. Address all problems identified in the POC. Provide educational information to the patient/family/caregiver regarding the patient\'s condition. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation according to company policy using a company-provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements Current OT License to practice in California - Required Experience in Home Health or Hospice care - 1 Year Required Current CA Driver's License Able to travel within a 15-25 mile radius on a regular basis.
