The role of the Care Manager is to ensure that individuals receive the most appropriate long-term care services in the proper setting based upon their individual needs. The Care Manager works cooperatively, coordinating service plans, and maintaining ongoing communication with the consumer, caregivers, informal and formal support. Ensure appropriate long-term care services are selected for each client. Coordinate service plans and maintain ongoing communication with consumers, caregivers, informal and formal support. Collaborate with multidisciplinary teams to support client independence. Monitor and evaluate client progress and adjust plans as needed. Geriatric Support Service Coordinator (GSSC) HESSCO has a full‑time position (35 hours per week) available for a Geriatric Support Service Coordinator. The GSSC conducts in‑home assessments and provides home‑based services coordination for clients under the Senior Care Options (SCO) plan, which combines health and social support services. Conduct in‑home assessments. Coordinate home‑based services under the SCO plan. Collaborate with the Aging Service Access Point (ASAP) and other stakeholders. Maintain accurate documentation and reports. Regional SHINE Program Lead The SHINE Program Lead manages the Regional SHINE Program, serving health insurance needs of everyone. This full‑time (35 hours per week) role coordinates training, direct service activities for volunteer SHINE counselors, and is responsible for all program reporting and data collection. Lead the SHINE Program operations across the region. Coordinate training and activities for volunteer counselors. Ensure accurate program reporting and data collection. Advocate for consumers requiring health insurance counseling. Protective Service Worker The Protective Service Program provides short‑term crisis intervention to older adults who need services to prevent, eliminate, or remedy the effects of abuse and neglect. The Protective Service Worker assesses allegations, conducts intakes, home visits, investigations, service planning, and crisis intervention. Assess allegations of abuse and determine service needs. Conduct in‑person intakes and home visits. Investigate incidents and coordinate crisis intervention. Develop and implement service plans. Registered Nurse HESSCO seeks a full‑time (35 hours per week) Registered Nurse. The RN participates in service planning and nursing assessments to determine medical eligibility for programs, performs in‑home nursing assessments, and works as part of the interdisciplinary team in discharge planning. Conduct nursing assessments of consumers. Determine medical eligibility for aging‑related programs. Identify personal care needs of home‑care consumers and nursing facility residents. Collaborate with interdisciplinary teams for discharge planning. Meal Site Manager – Supportive Housing Sharon Senior Housing Authority – Hixson Farm Road. Part‑time – Monday–Friday 10:30 am‑12:30 pm. The Meal Site Manager oversees the daily operation of the Congregate Lunch Program in a supportive housing program. Manage daily meal operations and serving procedures. Coordinate meal delivery to building residents and community elders. Maintain program records and documentation.
