Job Description
Title: Registered Nurse - Home Health

Schedule: 8hrs

Assignment Details:

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- CHPN certification (Preferred)
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program

Roles & Responsibilities:
- Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and/or their families on care, diet, medications, and home safety, employing effective teaching methods.
- Supervise and provide clinical direction to support staff to ensure quality care.
- Monitor and reassess pain management techniques, educating patients and families on effective pain management.
- Communicate effectively and serve as a liaison between patients, families, team members, and healthcare professionals.
