Aging Services Case Manager Empower Seniors & Families
Goddard Riverside
Goddard Riverside is seeking a Case Manager to support seniors in living independently in New York City. You will work with residents, families, and support systems to connect them with benefits and services that promote health, safety, and meaning.
The role involves helping with applications for Medicare, Medicaid, long-term care, SNAP, HEAP, and connecting to medical, in-home, and legal resources. 35 hours per week with a salary in the range shown.
