Field-Based RN Case Manager - Home Care in Chicago

BrightStar Care of Chicago and La Grange is seeking an experienced RN Case Manager to join our clinical team. This role is primarily field-based in the Chicago area with regular office time at 222 S. Morgan St., and extensive daily local travel (~80% field). You will conduct patient assessments and re-assessments, manage an active caseload, and serve as a clinical resource for field nurses, coordinating care plans and ensuring regulatory compliance. #J-18808-Ljbffr BrightStar Care
