Health/Human Service Case Manager II

Overview As a Health/Human Service Case Manager II, you will work in the Division of Social Services’ Change Report Center, a specialized team and call center where clients report changes in their circumstances. You will process these changes and determine ongoing eligibility for programs such as Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children’s Program (CHIP), Subsidized Childcare, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF). You will also provide counseling, case management, and referrals for services not offered by DSS, helping meet the diverse needs of Delaware’s vulnerable and low‑income residents. Essential Functions Interview client, family and/or professional sources to determine clients’ needs for a variety of social services which may include employment, job search and basic skills remediation. Develop a comprehensive cost‑effective service plan utilizing in‑house sources, referrals, contracted providers and/or institutions. Conduct on‑going monitoring and assessment of service delivery for optimum quality and efficiency, authorizing and recommending changes and adjustments as needed. Maintain comprehensive case records, summaries, statistical and narrative reports and complete required forms. Establish effective working relationships with clients, family, community and professional sources. Provide specialized casework and counseling services in such areas as protective, adoption and options counseling. Provide direct casework service through individual/group conferences, counseling and/or site visits. Supply information and responses to inquiries regarding services for clients, family, community and government agencies. May provide supportive casework to clients prior to, during or after hospitalization based on physician’s diagnosis and recommended treatment. Job Requirements Six months experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual’s human service needs. Six months experience in making recommendations as part of a client’s service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits. Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures. Six months experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data. Six months experience in narrative report writing. Conditions of Hire Applicants must be legally authorized to work in the United States. All new hires are required to report to the Statewide New Employee Orientation (SNEO) on their first day of employment. Sessions are held at designated locations in Kent or New Castle County. This position is a classification organized under an exclusive bargaining representative that has been elected by employees as their representative for collective bargaining and other applicable terms and conditions of employment, in accordance with Title 29, Chapter 59 and Title 19, Chapter 13 and 16. This position is covered by a collective bargaining agreement based on its individual terms.
