Physical Therapist (Home Health)

Location: Jersey City, NJ Job Type: Full-Time | Part-Time Schedule: Flexible Hours | No Weekends/On-Call Setting: Home Health (Outpatient - In-Home Care)

Position Overview

We are seeking a motivated Physical Therapist to provide one-on-one, in-home care to primarily geriatric and disabled patients. This role offers flexibility, autonomy, and the opportunity to deliver high-quality care without the pressure of high patient volume.

Key Responsibilities

• Provide personalized, one-on-one physical therapy in patients' homes
• Develop and implement individualized treatment plans
• Improve mobility, function, and pain management outcomes
• Maintain accurate and compliant documentation (Medicare Part B guidelines)
• Educate patients and caregivers on treatment plans and home exercises
• Travel between patient locations as assigned

Qualifications

• Licensed Physical Therapist (NJ)
• New graduates welcome (1 year of experience preferred)
• HPSO malpractice insurance is required
• Strong communication and time management skills
• Comfortable working independently in a home setting
• Knowledge of Medicare billing and the 8-minute rule is preferred
