LPN (Licensed Practical Nurse)

~ Minimum of 1 year experience as a Licensed LPN required with preference given for in home care.

~ Knowledgeable in regulations for VA, Medicaid and waiver programs for all services available in the homecare industry

~ Experience in maintaining clinical records in compliance with state, federal rules and regulations.

~ Strong interpersonal and customer service skills

~ Ability to handle multiple priorities simultaneously

Duties and responsibilities

Builds strong relationships with caregivers and clients/families
Educates the community, clients and families on services available
Must document and maintain clinical records in conformance with federal, state and Franchisor rules and regulations.
Ability to manage and coordinate a clients' needs using a case management approach under the direction of a registered nurse.
~ 30 - 35 Hours per week

~ $ Negotiable

~ $500 sign-on bonus after 90 days of employment
