Details Acute care hospital setting within the Case Management department Patient ratio of 1:25 Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM Variable 5-day workweek with department operations occurring 7 days per week Days off will vary and are not guaranteed to be consecutive Float to a nearby sister facility required Epic EMR experience required Requirements Active CA RN license required, pending license accepted if licensed by start date 2 years of recent acute medical Care Manager experience performing Care Coordination required within the last 3 months Prior travel experience required Trauma Level I and II experience preferred Epic charting experience required Experience in Case Management/Utilization Review including Appeals and Denials, Care Coordination, Disability Case Management, Discharge Planning, Disease Management, Needs Assessment/DME Ordering, Plan of Care, and Worker's Comp Case Management Familiarity with regulatory standards including CMS, Department of Health, DRG, HIPAA, OSHA, The Joint Commission/Core Measures/National Patient Safety Goals, and Workers Compensation ED, ICU, and Surgical Services experience preferred Two supervisory references covering 1 year of employment within the last 3 years required Full SSN and DOB required for consideration Additional Information Coordinate patient care, discharge planning, and case management activities within an acute care hospital Turquoise scrubs or business casual attire required No more than 7 days of RTO permitted Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) Local candidates accepted at a lower bill rate Radius requirement of 50 miles for local candidate consideration Former permanent staff must wait at least 3 months before returning as a traveler and must reside outside a 50-mile radius of the facility First-time travelers will not be considered for this position American Traveler Job ID P-750497.
