Business Development Liaison
Choice Health at Home is seeking an experienced and motivated Business Development Liaison with an established book of business in home health or hospice to help expand access to high-quality home health services throughout the community. In this role, you'll build relationships with physicians, hospitals, case managers, skilled nursing facilities, and other referral partners while serving as a trusted resource for home health solutions.

What You'll Do
- Develop and maintain referral relationships with healthcare providers and community partners
- Promote Choice Health at Home as the preferred provider for home health services
- Identify new business opportunities and referral sources
- Collaborate with clinical and operational teams to support patient admissions and care transitions
- Educate referral partners on services, programs, and patient eligibility
- Ensure compliance with all federal, state, and local marketing regulations
- Participate in community outreach, networking events, and professional organizations
- Meet and exceed monthly admission and growth goals
- Provide market feedback and recommendations for service expansion
What We're Looking For
- 2+ years of home health or hospice sales experience preferred
- Strong relationship-building and communication skills
- Experience using CRM systems preferred
- HomeCare HomeBase experience preferred
- Valid driver's license and auto insurance required
- Proven ability to meet sales and referral growth goals
