What Will You Do as a Physical Therapist? As a Physical Therapist at Pocopson Home , you'll evaluate and treat patients with a variety of mobility and functional needs while developing individualized treatment plans based on each person's abilities and goals. Your role will include: Performing comprehensive Physical Therapy evaluations Developing individualized plans of care and functional goals Addressing strength, balance, gait, transfers, endurance and functional mobility Providing therapeutic exercise and functional mobility training Developing fall-prevention and safety strategies Assessing the need for walkers and other assistive devices Monitoring patient progress and modifying plans of care when appropriate Supervising and collaborating with Physical Therapist Assistants Educating patients, families and caregivers Collaborating with the interdisciplinary care team Completing accurate and timely clinical documentation Most importantly, you'll help people work toward something that can completely change their quality of life: the ability to move more safely and independently.
