Enrollment Coordinator

Job Overview The PACE Life Program Enrollment Coordinator assists the team with screening and intake assessments, follows up on visits and Medicaid applications and redeterminations for eligible applicants, and coordinates center assessments. The role works closely with the County Welfare Office and Area Agency on Aging to enroll participants into the program. Responsibilities

• Assist in marketing and sales efforts including participation in health fairs and community presentations.
• Conduct one‑on‑one follow‑ups with interested parties.
• Provide quality customer service, participate in performance improvement efforts, and demonstrate teamwork and cooperation.

Qualifications High School Diploma required; Bachelor’s degree preferred. Valid NJ Driver’s License, registration and insurance required. Marketing or sales experience in healthcare, insurance or a similar environment preferred. Computer proficiency in Microsoft Word and Office Applications required. Bi‑lingual preferred. Minimum one year working in senior healthcare preferred. Performance Expectations Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position. Work Environment Operates within an interdisciplinary team model in an Adult Day Health Center and primary care clinic. Requires manual and fine dexterity, eye‑hand coordination, and the ability to lift/carry up to 30 pounds with proper body mechanics. Reporting Relationship This position reports to department leadership.
