Care Advocate

Community Health Worker (CCHW) – Remote (Florida Only) Pay: $23/hour | Full-Time Must reside in Florida We’re hiring an empathetic, self-directed Certified Community Health Worker (CCHW) to support Medicare patients in a fast-paced, growing program. This role is 90% remote with 2–3 local site visits per month (expenses reimbursed). Requirements: Active CCHW certification (required) 2+ years of community health, patient advocacy, or case management experience Ethernet (hard-wired) internet connection required Quiet, private home workspace (no public spaces) Strong time management and attention to detail What You’ll Do: Provide patient advocacy and resource coordination Connect members to community services Conduct occasional local outreach visits If you’re mission-driven and thrive in a virtual environment, we’d love to connect.
