Med Care Manager / QMAP Full-time, non‑exempt. Pay range $16.50–$20.00 per hour with a $1.00 per hour shift differential for 2pm–10pm and 10pm–6am shifts. Schedule to be discussed at interview. Responsibilities Administer and document medications per physician orders and medication administration regulations. Ordering medications from pharmacy 7 days prior to running out and ensuring timely delivery; notify Wellness Director within 24 hours if not received. Document medication administration in the resident's medical record immediately after giving medication; date and initial the bubble pack. Observe and report medication reactions and side effects to the licensed nurse and Wellness Director. Record vital signs prior to medication administration when ordered by Physician or Wellness Director. Administer PRN medications as prescribed. Report any medication error to the Wellness Director immediately. Assist in developing and reviewing care plans with interdisciplinary team. Maintain residents' privacy, respect, and dignity. Detect, correct, and report unsafe conditions that may harm residents. Ensure physical comfort, safety, and mental well‑being of residents. Assist residents with daily activities such as toileting, grooming, bathing, feeding, dressing, intake monitoring, hair and nail care, mobility, and response to calls. Qualifications Demonstration of Core Values: Love, Kindness, Honesty, Goodness, Fairness, Respect. License/Certification: Successful completion of state‑approved Medication Administration training, First Aid certification required by ALF regulations, and QMAP in CO. Education/Experience: Minimum high school diploma or equivalent; preferred experience in assisted living or memory care with medication administration and at least three years in senior living. Continuing Education: Must attend monthly in‑service education and training in community. English literacy and communication skills; ability to read and interpret safety rules, operating instructions, and procurement manuals. Knowledge of medication administration procedures, universal precautions, and resident safety. Ability to make resident‑first decisions and work within nursing limits.
