ProActive Physical Therapy is currently seeking a Full-Time Patient Care Coordinator for our Rancho Bernardo clinic, in San Diego, CA, where our "together we're better" philosophy creates a culture that makes coming to work enjoyable for everyone.Key ResponsibilitiesProvide a warm and professional welcome to all patients and visitorsDeliver exceptional customer service and patient support throughout the care processCommunicate effectively with patients, clinicians, referral sources, and internal departmentsGuide patients through visit preparation, electronic forms, paperwork, and appointment expectationsEnsure accurate patient registration and data entry within the EMR systemReview patient charts for completeness and accuracyManage patient scheduling and appointment optimization to maintain clinic efficiencyCoordinate communication with referral sources and assist with follow-upsAnswer and triage incoming phone calls in a professional and timely mannerCollect copays, patient balances, and verify insurance informationPerform scanning, faxing, and other administrative dutiesSupport clinicians and assist with patient care coordination when neededMaintain organized documentation and ensure compliance with office proceduresPreferred Qualifications1–3+ years of experience in a medical front office, patient coordination, or healthcare administration roleExperience in Outpatient Rehabilitation, Physical Therapy, or a specialty clinic strongly preferredStrong knowledge of insurance benefits verification and common insurance terminologyProven ability to manage high patient volumes while maintaining accuracy and professionalismExcellent customer service, communication, and interpersonal skillsStrong organizational, multitasking, and problem-solving abilitiesComfortable working collaboratively within a team-oriented clinical environment
