As a member of the rehabilitation team, you will help ensure the health, well-being, independence, and quality of life of residents through comprehensive occupational therapy services.This position includes a schedule of weekdays, with some weekend and holiday coverage.Key ResponsibilitiesInitiate skilled occupational therapy treatments in accordance with physician orders and maintain required physician communicationComplete and maintain written evaluations, progress notes, MDS documentation, and discharge summariesCollaborate with interdisciplinary healthcare team members to address residents' rehabilitation and functional maintenance needsParticipate in Resident Care Planning meetings and attend in the absence of the Director of Occupational Therapy when neededMaintain statistical records of resident treatments and therapy outcomesAssist with in-service education and training programs upon requestRecord, update, and monitor resident care plansSupport residents in achieving their highest level of independence and functional abilityQualificationsGraduate of an accredited Occupational Therapy programCurrent state Occupational Therapy license requiredTwo years of occupational therapy experience preferredInterest in working with geriatric and subacute populations using a team-based approachAbility to supervise and direct others when necessaryStrong verbal and written communication skillsProfessional maturity, diplomacy, and the ability to work collaboratively with residents, families, and staffCompassionate and patient-centered approach to careAbility to work effectively with geriatric and chronically ill residentsCommitment to maintaining established standards of clinical excellenceWhy Ticonderoga?Located between the breathtaking shores of Lake George and Lake Champlain, Ticonderoga offers a unique blend of natural beauty, rich history, and year-round outdoor recreation.
