Overview 5,000 sign on bonus available. 2,500 relocation bonus (greater than 50 miles) (external candidates only). The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages, modifying the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need. Qualifications Certifications: Proof of Auto Insurance (varies); Basic Life Support (BLS) within 30 days—American Heart Association (AHA); Driver's License—Secretary of State (SOS); Licensed Physical Therapist (PT)—Illinois Department of Financial and Professional Regulation (IDFPR) Education: Bachelor’s Degree in Physical Therapy (required) Work Experience: (none specified in posting; responsibilities indicate required experience) Responsibilities Document comprehensive detail about patient visits so that following staff can determine exactly what and how care was rendered, with a clear plan for the next contact. Educate and document discharge planning prior to discharge, including communication with physician, insurance source, outpatient visits, and community resources; provide discharge goals promptly and ensure goals are closed as met or changed as patient condition progresses. Collaboratively communicate and initiate case conferencing as necessary, documenting interactions with other disciplines, MDs, care coordinators, insurers, etc. Communicate requirements for the collaborative team and caregivers, coordinate assignments with the scheduler, prepare written instructions for the assistant, and supervise the assistant during in‑home visits. Administer treatment involving application of physical agents such as heat, cold, light, water, electricity, traction, and massage therapy. Evaluate, fit, and adjust prosthetic and orthotic devices, recommending modifications to the orthotist or prosthetist. Plan and administer prescribed physical therapy treatment programs to restore function, relieve pain, and prevent disability, injury, or loss of body part. Perform patient assessment tests, measurements, and evaluations—range of motion, manual muscle tests, gait and functional analyses, and body part measurements; record and evaluate findings to aid in establishing or revising treatment programs. Review and evaluate physician or podiatrist referrals and patient medical records to determine if physical therapy is required. Demonstrate comprehension and adherence to regulatory requirements and home‑care policies regarding practice, documentation content, and timelines. Complete functional assessments and documentation per comprehensive OASIS assessment. Assess homebound status of patients. Complete comprehensive physical therapy assessment in the patient’s home and develop a plan of care. Communicate with patients to schedule appointment times and manage daily schedule. Assess home safety and recommend equipment to improve patient safety. Educate family and caregivers on recommended home exercise programs. Perform wound care within scope of practice and according to the plan of care and department policy. Compensation and Benefits The compensation range for this position is $36.74 per hour – $63.19 per hour. This represents a good‑faith minimum and maximum range for the role at the time of posting by Carle Health.
