CTI Care Manager — Crisis Recovery & Community Supports
AHRC New York City Camping
AHRC New York City is seeking a Care Manager for the Critical Time Intervention Program to develop trusting relationships with participants and coordinate supports across community resources. The CTI Care Manager assesses needs, develops plans, makes referrals, and connects people to needed services using in-person, in-home, or community-based approaches while following CTI principles. #J-18808-Ljbffr AHRC New York City Camping
