We are looking for a Case Manager- Qualified Mental Health Professional Crisis Care Counselor Mobile Lead who demonstrates this mission and wants to work for an organization that makes a difference in the community. Schedule-Specific Hours: 6a-6p Day: Mon, Tues, Sat, rotating Friday What you will be doing? Providing crisis intervention, de-escalation, emotional support and suicide risk assessment to callers in crisis via phone through AID’s 24/7 crisis line. Manage and assess incoming Mobile Crisis referrals from 988, local police departments, and other community services via phone through AID’s 24/7 crisis line. Provide in-person Mobile Crisis Response with a recovery support specialist anywhere in Kendall and Southern Kane County when a person is experiencing a mental health crisis that cannot be de-escalated over the phone. Complete in-person assessments to determine suicide risk, level of care need, and access to support. Work with the person in crisis and their collaterals to complete a safety plan. Collaborate with a team of service providers within AID and throughout the social service community to ensure that all client’s needs are being met to promote and maintain community stabilization. Maintain documentation required for crisis intervention services provided on the phone and in the community Provide supervision and support to all recovery support staff to ensure maintenance of CRSS certification process Assist Program manager with carrying out day to day operations of the program as necessary including but not limited to, staff training, administrative responsibilities, case management and community relations Assist with providing supervision and support to all Crisis Care System staff when providing crisis intervention on the phone and in the community. Attend to scheduled work hours and be flexible to meet client and program needs, as assigned by Program Manager or Director. Respond to calls from shift start to shift end. Respond to calls upon being dispatched Provide Crisis Line Coverage when not providing mobile response. Provide effective individual crisis counseling and crisis intervention assessments. Consult with family members, case manager, previous and existing service providers, and other outside resources as appropriate. Use intervention tactics to reduce symptomology Responsible for providing independent program coverage when scheduled or in emergency situations. Complete clinical assessments that meet the requirements of program funders and applicable administrative rules. Provide effective telephone counseling service through active and empathetic listening and motivational interviewing. Complete required documentation on a timely basis Billing within the next 2 shifts, AND prior to program billing deadlines. Complete MCRT Daily logs, Dispatch logs, 988 follow up and iCarol contact logs before the end of the working shifts. Attend clinical supervision a minimum of once per month & as assigned by program manager/s and director Must meet monthly minimum predetermined productivity standards when applicable Complete all necessary assessments including the crisis intervention and suicide assessment and required documentation including billing notes in accordance with Rule 132 standards Develops and implements crisis safety plans and resource follow up in collaboration with the client, other providers, and other parties as identified by the client, following the standards of Rule 132, payers, and accreditation bodies Provide case management follow up for clients to support linkage to services Maintain records of individuals served on the MCR-T in accordance with Rule 132, payers, and accreditation bodies. Assist MCR-T Lead in conducting regular quality assurance of MCR-T records. Maintain open and effective communication with agency and provider staff, family, and others in response to client’s identified needs, and client assistance/service needs. Navigate Cx360, have a working knowledge of opening clients to programming, ensuring client profile is completed, attaching necessary paperwork (IM-CAT, CSSR, consents/releases and safety plans) and uploading encounters and notes. Navigate crisis line documentation service, working knowledge of submitting documentation, finding frequent callers, and submitting sunshine documentation. Recognize emergency situations and take appropriate action. Contact Manager and Director per procedure. Complete necessary paperwork correctly (incident reports, petitions, case notes) Participate in training and development opportunities as assigned. Promote awareness of mental health, both internally and externally. Assist manager in ensuring appropriate staffing patterns; including, but not limited to 24/7 coverage, provide program coverage when needed (including overnight, evening, weekend and holiday hours). Assist manager in staff orientation to job, programs, Agency and provide ongoing training. Utilizes a team approach to provide direction to MCR-Team members, including triaging client needs and assigning tasks to ensure client needs are met. Assist manager in completion of all reports in a timely manner (Productivity MRO Reports, Outcome Measures, New Employee Monthly Billing Review, etc). Train all Crisis Care staff to complete skills and client programming that is in line with program requirements. Responsible to act as an extension of management, ensuring CCS staff receive consistent guidance and information consistent with program expectations and directives from management/leadership Monitor Crisis Care Staff in daily assignments, hold staff accountable to completing assignments daily and turning in daily logs Track staff trainings to ensure compliance and assist staff to schedule trainings as needed Promotes awareness and respect for the diversity of our clients,employees, families and other stakeholders. Obtain input from consumers/stakeholders on a regular basis to improve services. Review and implement updated policies and procedures from the Agency and Program procedure manual. Acts as a role model for all staff in implementation of program standards, execution of job responsibilities and associated skills, and demeanor toward clients and peers. Review and ensure the proper completion of the following: Staff Daily Logs, Incident Reports, Crisis Intervention documentation, Safety Plans, IM-CAT, Case notes. Assist in quality assurance tasks as assigned. Assist program manager with providing QMHP consult and supervision to MHP Crisis Counselor staff when engaging in Mobile Response. What will you bring to the table? Education: Master’s degree in a human services field (QMHP) required. Experience: Two or more years of experience in behavioral health/crisis services preferred. Bilingual-Spanish Preferred, Knowledge of the DSM V or most current edition and experience with Medicaid Rule 132 preferred. Physical: Position requires ability to communicate in-person, telephone and in group settings. Requires frequent standing; walking, navigation of stairs on a regular basis, Ability to provide services in clients’ homes and in the community. Ability to drive and work overnights is required Equipment: Position operates in a professional office environment and off-site in various community settings. Role routinely uses standard office equipment such as computers, phones, copiers, and filing cabinets Additional Requirements: Flexibility to work evening/overnight/weekend/rotating holidays Ability to work rotating on-call shifts.
