Full-Time Adult Mobile Crisis Case Manager (1st Shift)

Overview Job Details: Entry level position in Huntsville, AL. Full-time, Monday–Friday, 7:00am–3:00pm. Position category: Nonprofit - Social Services. Education requirement: Human Services Related B.S./B.A. LVL degree. Travel up to 50%. Responsibilities Provide outpatient case management services to the seriously mentally ill population. Provide direct services to severely mentally ill clients to address unmet needs and provide information about the continuum of care. Provide training on money management to assist clients in obtaining and maintaining psychiatric stability in residential settings. Provide follow-up progress reports as needed by the client and treatment team members. Evaluate client’s residential stability and assist those in need of a more stable environment. Provide Basic Living Skills training as needed to prepare clients for less intensive levels of care within the community. Assist clients with obtaining medication; monitor medication compliance and awareness of common side effects; ensure regular adherence to prescribed regimens. Provide traditional case management services and collateral services such as transportation to appointments and referrals to agencies. Maintain high-quality client documentation and electronic charting. Monitor care and services in residential settings and develop a positive working relationship with Residential Care managers/owners to meet client needs. Develop and implement Individual Service Plans (ISPs) including goals, resources, and interventions; review progress every three months and revise ISPs as needed. Review SUN-R at six months after initial assessment and then at 12-month intervals; revise client goals as progress occurs. Assist mentally ill clients through crisis situations and coordinate with other professionals or caregivers as needed. Perform incidental care to families of the mentally ill and support family members’ understanding of mental illness. Complete other duties as assigned. Qualifications Bachelor’s degree in Psychology, Counseling, Human Services or social sciences field. Acceptable 3-year driving record and reliable transportation. Excellent verbal and written communication skills. Successful completion of State of Alabama Adult and Child/Adolescent Case Management Training. Knowledge of local community resources and agencies.
