What Will You Be Doing?
As a Physical Therapist with Gallagher Home Health, you will evaluate, plan, coordinate, and provide patient care based on the Physicians Plan of Treatment. Youll have the chance to make a direct impact on patients lives while working in the comfort of their homes.
Responsibilities
- Provide comprehensive patient care through assessment and treatment planning in home health settings.
- Utilize knowledge of physiology and anatomy to develop effective physical therapy interventions.
- Collaborate with healthcare teams to create and implement care plans, ensuring continuity of care.
- Educate patients and families on therapeutic exercises and discharge planning.
- Maintain accurate medical records and documentation in accordance with regulations.

What Were Looking For:
- Active Physical Therapist license in the State of Pennsylvania
- At least 1 year of recent experience (Home Health experience preferred)
- Valid Pennsylvania Drivers License with no restrictions and a car with required insurance
- Satisfactory Criminal Record Clearance
