• Conduct thorough assessments of clients experiencing a range of social, emotional, and psychological challenges to identify personalized intervention strategies.

• Provide both individual and group counseling sessions, utilizing evidence-based therapeutic techniques and fostering a safe environment for sharing and healing.

• Develop and implement targeted service plans aligned with clients' goals, regularly reviewing and adjusting plans based on progress and needs.

• Act as an advocate for clients, ensuring their rights are upheld and facilitating access to critical resources and services.

• Collaborate with community organizations, educational institutions, and government agencies to strengthen service delivery and enhance community programs.

• Engage in program evaluation, grant reporting, and the development of new initiatives aimed at addressing social justice issues and improving community health outcomes.

• Participate in team meetings, ongoing training, and outreach efforts to effectively promote our programs and refine best practices.

This role focuses primarily on individuals and families facing grief, end-of-life issues, chronic illness, and other significant life changes associated with hospice care. As an LCSW, you will provide critical emotional and psychological support, assisting clients in navigating their challenges while advocating for their dignity and quality of life.

• A Master's degree in Social Work from an accredited institution, reflecting your comprehensive training in clinical social work practices.

• Current state licensure as a Licensed Clinical Social Worker (LCSW), ensuring compliance with legal and ethical standards.

• Strong communication, interpersonal, and organizational skills, with an emphasis on empathy and ethical considerations in clinical work.

• Demonstrated understanding of trauma-informed care principles, social justice advocacy, and community resource navigation.
