An established outpatient primary care practice is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join a collaborative healthcare team serving a diverse patient population in Central California.

Key Responsibilities

• Provide comprehensive primary and preventive care to patients of all ages.
• Evaluate, diagnose, and manage acute and chronic medical conditions.
• Conduct physical examinations and wellness visits.
• Order and interpret diagnostic tests and laboratory results.
• Develop evidence-based treatment plans.
• Prescribe medications and therapies within scope of practice.
• Coordinate referrals and follow-up care with specialists.
• Educate patients on disease prevention and healthy lifestyle choices.
• Maintain accurate and timely electronic medical records.
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams.

Qualifications

Required

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Active California NP or PA license, or ability to obtain licensure.
• Strong clinical assessment and patient management skills.
• Excellent communication and interpersonal abilities.

Preferred

• 1+ year of experience in Family Practice, Primary Care, Internal Medicine, Community Health, Urgent Care, or Outpatient Medicine.
• Experience managing chronic diseases and preventive care services.
• Bilingual skills are a plus but not required.
