Social Worker – Behavioral Health
Schedule: Shift: Days, 07:00–19:30 Every other weekend (Saturday & Sunday)
Requirements:
- Required licensure: Licensed Mental Health Counselor, Marriage & Family Therapist, or Independent Clinical Social Worker (LICSW)
- Minimum 1 year of clinical experience
- Experience conducting crisis/risk assessments for adults and pediatrics
- BLS certification required
- Familiarity with DCF processes, adult/child protective services
- Proficiency in Epic charting system
Roles & Responsibilities:
- Conduct crisis and risk assessments for both adult and pediatric populations.
- Perform biopsychosocial assessments and manage involuntary holds as necessary.
- Coordinate care and plan for discharges by connecting patients with appropriate rehabilitation, long-term care, and community resources.
- Provide individual, marriage, and parental counseling to support emotional and mental health needs.
- Document all processes accurately to support care coordination and compliance with applicable laws and regulations.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
Compliance / Notes:
- Compliance documents are due the Monday prior to start.
- Must provide last 4 digits of SSN.
- Requested time off (RTO) must be noted at the time of submittal.
- References: 2 required covering 1 year within the last 3 years.
- Must disclose any prior employment within the organization (facility & dates).
- Pre-start modules are required, with an average completion time of 16 hours.
- Travelers may be expected to float to sister facilities; refusal may affect assignment.
