• Shift: Monday Friday, 8:00 am 5:00 pm
• Duration: Minimum 2 weeks per month (prefer full-time)

• License/Certs: Active Medical License, DEA, BLS
• Experience: Prior Geriatric Medicine experience preferred

• Hourly Pay Rate: $1.00/hour

What You ll Do:

• Provide outpatient primary care to seniors age 55 and older, with a focus on geriatric health management
• Conduct evaluations and coordinate care for patients in the clinic and community
• Participate in off-site patient visits in assisted living, SNFs, or personal residences
• Collaborate with a team of nurses and interdisciplinary staff to deliver comprehensive care
• Manage patient records using electronic medical records (EMR) systems
• Ensure medication management, preventive care, and chronic disease management
• Work 2+ weeks per month, with a flexible schedule of 3-4 days per week

What You Need:
Required:

• Valid active Medical License in Oregon
• DEA certification
• BLS certification
• Geriatric or Internal Medicine experience

Preferred:

• Prior outpatient and community-based care experience
• Ability to work independently and as part of a team
