Job Opportunity Position Overview We are seeking a dedicated and experienced Home Health Physical Therapist Assistant (PTA) to provide exceptional patient care in a home health setting. This position involves working closely with patients to help them achieve their rehabilitation goals in various areas, including Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont. Key Details
- Shift: Day 5x8-Hour (08:00 - 16:30)
- Start Date: ASAP (Compliance takes approximately 2 weeks)
- Guaranteed Hours: 36 hours weekly (with productivity requirements)
- Patient Ratios: 1:1 (may visit up to 6 patients per day; 27 productivity points standard)
- Weekends: Optional patient visits available to meet productivity
- On-Call Requirement: No
- Local Candidates: Accepted

- Outpatient Clinic
- Rehabilitation Hospital
- Skilled Nursing Facility (SNF)
- Mobility Assistance (e.g., crutches, gait belt, walkers)
- Post-operative rehabilitation
- Cognitive disorders and neurological conditions (e.g., Stroke, Multiple Sclerosis, Parkinson's disease)
- License: Must possess a valid California license
- Certifications: Current BLS certification required
