Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Ohio Physical Therapist license required to start the assignment.
BLS (Basic Life Support) certification from AHA or ARC may be required.
Home health experience required. Must be proficient in home health evaluation, treatment planning, patient and caregiver education, and documentation of skilled services.
Ability to meet productivity expectations: minimum of 35 points per week.
Reliable transportation and ability to travel within assigned territory (Territory: Purple) to visit patient homes.
Previous experience managing a caseload in home health settings preferred.

Assignment Details

Facility Type: Home Health.
Shift: Days.
Shift Pattern: Monday - Friday, 8-hour shifts (40 hours per week).
Assignment Length: May through August (assignment length listed).
Must meet minimum weekly productivity expectation of 35 points.
Territory-based caseload. Candidate must travel within the assigned territory to patient homes.
