If you're passionate about clinical pharmacy, patient engagement, and improving healthcare quality, this role offers a rewarding opportunity to apply your expertise every day. \n Accountabilities Deliver comprehensive Medication Therapy Management (MTM) services, including Comprehensive Medication Reviews, Targeted Medication Reviews, adherence interventions, cost-saving recommendations, and transitions of care support. Conduct proactive outreach to patients, explain the benefits of MTM services, address concerns, and encourage participation using effective communication and motivational techniques. Respond to inbound patient inquiries, assess medication-related needs, and provide appropriate clinical guidance in accordance with established protocols. Evaluate medication regimens for appropriateness, effectiveness, safety, adherence, dosing accuracy, and potential therapy gaps while developing evidence-based recommendations. Collaborate with prescribers and interdisciplinary healthcare teams by communicating clinical interventions and documenting recommendations to support coordinated patient care. Educate patients and caregivers on chronic disease management, medication adherence, preventive care, and treatment plans to improve long-term health outcomes. Maintain accurate, timely, and compliant clinical documentation while adhering to applicable pharmacy regulations, CMS requirements, HIPAA standards, and internal quality guidelines. Requirements Bachelor's Degree in Pharmacy or Doctor of Pharmacy (PharmD) from an ACPE-accredited institution or equivalent international pharmacy education. Current pharmacist license in the practicing state(s), with the ability to obtain additional state licenses if required. Completion of a Medication Therapy Management (MTM) certificate program. Minimum of 1 year of professional pharmacist experience. Strong clinical knowledge in medication therapy management, chronic disease management, medication safety, adherence strategies, and evidence-based therapeutic decision-making. Experience communicating effectively with patients, caregivers, prescribers, and multidisciplinary healthcare teams. Excellent verbal and written communication skills, including the ability to explain complex medical information clearly and compassionately. Strong organizational skills, attention to detail, and the ability to work independently in a remote environment while managing multiple priorities. Proficiency with electronic clinical systems, Microsoft Office applications, and telehealth technologies. Board certification in Ambulatory Care, Geriatrics, Pharmacotherapy, MTM, or another pharmacy specialty, ASHP residency or fellowship training, multilingual proficiency, motivational interviewing experience, and multi-state pharmacist licensure are considered strong advantages.
