Job Description:
Home Health Hospice: RN Case Manager
**Must have at least 12 months of home health experience within the past 3 years
HCHB experience required. May accept an HCHB training through candidate's agency prior to start.
20 to 25 visits per week and 40HRS/ per week guaranteed
Monday - Friday, 40 hours per week
Area of coverage is Southside Richmond
