We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Care Pavilion Nursing and Rehab in Philadelphia, Pennsylvania. As an LPN at Care Pavilion, you will be responsible for providing high-quality patient care while working collaboratively with our interdisciplinary team of healthcare professionals. Your day-to-day responsibilities will include:

• Administering medications and treatments as prescribed by physicians
• Monitoring and recording patient vital signs and medical information
• Assisting patients with activities of daily living (ADLs) such as bathing, dressing, and grooming
• Providing emotional support and education to patients and their families
• Participating in patient care conferences and developing individualized care plans

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse or physician
• Maintain accurate and detailed records of patient treatments and progress
• Communicate effectively with patients, families, and healthcare team members
• Participate in ongoing education and training to stay current with industry best practices and regulatory requirements
• Collaborate with other healthcare professionals to achieve patient-centered care goals

Requirements:

• Current LPN license in the state of Pennsylvania
• High school diploma or equivalent required; associate's or bachelor's degree in nursing or related field preferred
• Minimum 0 years of experience in a long-term care or subacute setting; experience with electronic medical records a plus
• Ability to lift, transfer, and reposition patients as needed
• Strong communication and interpersonal skills
