Your Role

We’re seeking a compassionate, experienced Home Health Physical Therapist to join our high-performing team. The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy.

What You’ll Do

· Conduct OASIS assessments and initial start-of-care visits to establish individualized plans of care

· Evaluate functional limitations and develop personalized therapy goals and interventions

· Guide patients through therapeutic exercises and mobility programs to maximize independence

· Educate patients and caregivers on safe home exercise programs and fall-prevention techniques

· Collaborate closely with physicians, nurses, and other care-team members to coordinate care

· Monitor patient progress, update care plans, and maintain thorough, compliant documentation

· Ensure compliance with all Medicare, CMS, and home health standards

What We’re Looking For

· Current Florida Physical Therapist (PT) license (required)

· Previous home health experience is strongly preferred but not required

· Excellent communication, time management, and documentation skills

· A compassionate, dependable, and professional attitude
