In this impactful role, you'll conduct annual wellness exams and vital screenings that enhance the quality of patient care. You'll perform thorough health assessments, conduct non-invasive screenings, and offer personalized recommendations for disease management. Role : Open to Nurse Practitioners and Physician Assistants Location : Rotation between Mystic, Waterford, Plainfield, Norwich, and New London. Setting : 100% Outpatient Primary Care Schedule : Full-time, weekdays only. In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists. Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the providers practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician. Qualifications Requirements: Licensure : Unrestricted State of Connecticut Medical License (or eligible) Training : Nurse Practitioner (Family, Adult/Geri Nurse Practitioner) or Physician Assistant Experience : New graduates are welcome to apply!
