Hospice Nurse Case Manager — Home-Care Leader
Luminary Hospice
Luminary Hospice seeks a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes community health education and coordinating services with the interdisciplinary team. The successful candidate will hold an accredited nursing degree, current Texas RN licensure, and CPR certification, with preferred hospice/home care experience.
