Registered Nurse - Homecare Coordinator

Park Nicollet is looking to hire a Homecare Coordinator to join our Homecare team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.


Position Summary:

The Homecare Coordinator is responsible for planning and coordinating the delivery of individualized care and comprehensive service to patients and families. The Homecare Coordinator supports the following major Homecare processes: admission, discharge, referral, delivery of care, complaints, staff orientation, training and development, and billing.


Work Schedule:

M-F 8am-4:30pm, every 6th-8th weekend.

Required Qualifications:

• Education, Experience or Equivalent Combination:
  • Minimum 3 years' experience in home care
  • Registered nurse: Associate or Diploma degree
• Licensure/ Registration/ Certification:
  • Current license to practice as a professional nurse in Minnesota.
• Knowledge, Skills, and Abilities:
  • Excellent written and oral communication skills and ability to speak to groups.
  • Committed to the philosophy of customer service.
  • Must possess knowledge of reimbursement regulations.
  • Ability to problem solve and interface with primary customers.
  • Ability to relate to allied health professionals and to work with and through others.
  • Demonstrate knowledge of age-related growth and development principles necessary to provide appropriate services and assure safety of patients.

Preferred Qualifications:

• Education, Experience or Equivalent Combination:
  • PHN preferred.
  • Previous coordinator/supervisory experience preferred.
